Eligibility

  • License-exempt childcare providers who cared for at least one subsidized child during all three months of Aug, Sept and Oct of 2021 are eligible to apply for a one-time payment of $1,500.

Steps to Submit your Forms and Apply for Funding!

  1. Check to see if you already received an email with your unique invitation and link to apply!

Search your inbox for an email from: [email protected]

Subject of the email “Instructions to Receive Your Supplemental Rate Payment”

*If it’s not in your inbox, check both your ‘junk’ and ‘spam folders’*!

  1. Once inside the email, click on the personalized link under “Instructions to Receive Payment.” This link will take you to the form you must fill out.
  1. Complete your form!

Helpful tips for completing the form:

  • Carefully read the instructions on the form.
  • Go slowly when inputting and verifying your information.
  • Which Federal Tax Classification should I select?
    • Most license-exempt providers will select: “Individual/Sole Proprietor” unless you are incorporated or are an LLC business.
    • If you do not have an ITIN or EIN and use a social security number, you will more than likely select “Individual/Sole Proprietor.”
  • If you select ACH (Direct Deposit) as the payment type, make sure to double-check that the routing and account numbers are correct.
  • When submitting the Social Security Number, ITIN or EIN, it should be 9 numerical characters with no dashes or spaces.
    • You must submit either an SSN/ITIN or an EIN for payment, not both. This information identifies whom the reportable income will be taxable.
    • Please do not submit any other number types within the SSN/ITIN or EIN fields.
  • The information provided in the form submission will auto-populate the W-9.
    • You will not need to upload or submit a W-9 separately.
  • After the form is submitted, there is no further action required and the unique link will be disabled.

  • If you call the helpline and are informed that you are not showing up on their list,
    • You may be asked to demonstrate proof of eligibility and submit an Eligibility Review Form.
      • This form asks you to submit images (a scan or photograph) of your payment check stubs or direct deposit remittance slips (from the agency who issues your child care reimbursements) to show proof that you cared for at least one subsidized child in August, September, and October 2021.
